Please fill out one form for each competition and the names of the participants for that particular event.  If registering soccer, please provide team name and fill as many names as possible either participant or as an alternate.

*Please fill out one sheet for each competition your team is competing in (5 required to win the overall trophy, exception of the general manager event).
